Applicant Verification Process
University employment policy for regular staff positions requires that when a department identifies the applicant it wishes to hire, the department must take the following actions BEFORE it extends an offer.
The department must verify at least two employment history references from the Employment Application. The department will complete one Employment History Verification for each reference.
If college work or a conferred degree is either a required qualification or a selection factor for the vacancy, the department must require the applicant to provide copies of official transcript(s) for verification purposes.
If a professional certification and/or affiliation is either a required qualification or a selection factor for the vacancy, the department must require the applicant to provide copies of documents to verify such certification and/or affiliation.
Completed Employment History Verification forms and copies of transcripts and professional certifications and/or affiliations must be returned with the Applicant log.
An individual's appointment CANNOT be effective and the department CANNOT allow the individual to begin work UNTIL this documentation has been provided.
INSTRUCTIONS FOR EMPLOYMENT HISTORY VERIFICATION FORM:
Complete Applicant and Employer sections, entering date and time contact is made with the Employer.
The verification section must be completed. For each of the required items, read to the Employer the information provided by the applicant on the Employment Application form. Ask the Employer to verify. If the Employer does not verify any item(s), use the Comments/Explanations section to explain.
If the Employer requests a release of information authorization from the applicant, you may quote the authorization signed by the applicant on the Employment Application.
If the Employer requests a written authorization from the applicant, you may copy the authorization section of the applicant's Employment Application and mail or fax it to the Employer.
You may ask the optional questions if you desire.
When complete, print your name, sign and date the form.
